Claro CDI (Clinical Documentation Improvement) is a holistic approach to accurate documentation that results in enhanced and sustainable CDI processes to fuel your mission. The solution focuses on getting the record right across the continuum of care: Inpatient, Outpatient and Physician Office.

We can help you achieve real and sustainable value. For a hospital or health system to appropriately reflect quality of care and achieve appropriate reimbursement, the patient acuity in all settings must be accurately demonstrated. All patients and all encounters matter.

We bring the perspective and experience of having worked with hundreds of hospitals, including some of the largest academic medical centers and health systems in the country, as well as many community-based hospitals. We use this expertise to help each of our clients achieve their objectives.

Our focus is on the balance of people, process, and technology needed to facilitate extraordinary performance.

Our Areas of Expertise

We partner with hospitals and health systems to enhance and expand existing CDI programs to appropriately reflect quality of care and capture appropriate revenue for the complexity of patients treated. We find that a balance of people, process, and technology is essential to achieve peak performance.

The goal is to create a more robust and coherent medical record across the care continuum. Enhance collaboration and communication among clinicians. Capitalize on opportunities to capture additional documentation to reflect the true acuity of patients and complexity of care provided.

We are leaders in expanding CDI programs to the hospital facility outpatient setting. This includes all outpatient services: Emergency Department, Observation, Operating Room, GI/Endo. Cath Lab, Cancer Center, etc. We have partnered with hospitals nationally to review and implement enhanced processes and systems to improve the documentation, coding, and charging in the facility outpatient setting for more accurate reflection of the resources consumed and accurate reimbursement for the services rendered.

We collaborate with Physicians to educate, enhance, and improve their clinical documentation to improve Evaluation and Management (E/M) code selection and reflect accurate patient risk (including Hierarchical Condition Categories – HCCs), which is vital to individual providers and hospitals alike.
