Chicago IL, June 28, 2019  – JourneyCare, Inc., the largest nonprofit provider of palliative, supportive and end-of-life care in Illinois, completed an acquisition this month of Northwestern Memorial HealthCare’s (NMHC) home health and hospice business units located in Carol Stream, DeKalb, and Lake Forest, IL.
